  • Initiate, implement, and oversee disease management and education programs to address education and self-care needs identified by the patient and family.

Clinical Collaboration & Documentation
  • Communicate and collaborate with a diverse healthcare team to inform plans and actions, and provide teaching that benefits patients, families, and the organization.
  • Conduct interdisciplinary collaboration with physicians on patient management, including evaluating and revising therapeutic plans and managing high-risk, unstable, or complex patients.

Who You Are

  • Confident diagnosing and treating the full range of pediatric conditions — from upper respiratory infections and asthma to ear, eye, and nasal infections, minor cardiac and allergic conditions, eczema, and beyond.
  • A strong communicator who can relate constructively to patients, families, and colleagues from diverse backgrounds.
  • Committed to the military mission and motivated by meaningful service to families who sacrifice alongside those who serve.

Qualifications

  • Graduate degree or higher in nursing from an accredited educational program.

Skills & Competencies
  • Proficient in Electronic Medical Records (GENESIS), Microsoft Office, Medical Readiness Tracking Applications, and Workload Tracking Systems (DMHRSi).
  • Demonstrated ability to read, write, and speak English effectively for clinical communication with patients, families, and healthcare team members.

Work Authorization & Health Requirements
