Overview
Social Worker – Medical Social Work, travel assignment in Everett, WA. Start date 05/26/2026 for 13 weeks. Weekly pay range $1,286–$1,358. Guaranteed hours 72 per 2 weeks. Shift: 4x8 Days. Setting: inpatient Short Term Acute Care unit within a Level II trauma center.

Responsibilities
- Conduct psychosocial assessments for adult and geriatric inpatients
- Develop and implement discharge planning and care coordination plans
- Facilitate referrals to home health, rehabilitation, long-term care, and related services
- Collaborate with an interdisciplinary team (PT, Pharmacy, Interpretation, Social Services, Transportation) to support patient care
- Document assessments, plans, and progress in EPIC; maintain accurate care documentation
- Provide patient and family education, counseling, and crisis intervention as needed
- Advocate for patient rights and coordinate smooth transitions of care to appropriate services

Requirements
- Active Washington state social work license (LMSW/LCSW accepted; LICSW/LSWAIC/LASW/LSWAA acceptable per posting)
- Pending license accepted with WA license in hand at submittal per client guidance
- Master’s degree in Social Work (MSW) or equivalent
- Minimum 1 year of relevant experience in acute care, discharge planning, or care coordination
- BLS certification
- Ability to work in an inpatient acute care setting and to float within the hospital system
- EPIC charting experience preferred
