Responsibilities
- Conduct comprehensive PT evaluations, interpret findings, and establish individualized treatment plans.
- Implement evidence-based physical therapy interventions to improve mobility, strength, balance, and functional outcomes.
- Reassess progress regularly, update goals, and modify treatment plans as needed; document progress accurately in patient records.
- Educate patients and families on home exercise programs, safety strategies, and activity modification; collaborate with the care team for discharge planning.
- Communicate effectively with physicians, other therapists, and support staff; ensure adherence to safety, privacy, and documentation standards.

Requirements
- Active Physical Therapist license in Minnesota
- Doctor of Physical Therapy (DPT) degree or equivalent
- Eligibility for travel assignment and credentialing process
- Ability to work in a fast-paced rehabilitation setting and maintain professional documentation
- Strong communication, teamwork, and patient-care skills
- Availability to work Monday through Friday, 8:00 a.m.–4:30 p.m. and meet 40 hours per week
