Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives.
- Serve as co-leader in an interdisciplinary approach to patient groups.
- Document patient progress and communicate with the interdisciplinary team; ensure safety and regulatory compliance.
- Deliver therapy interventions in a home-based hospice setting, tailoring plans to patient and family goals.
- Educate patients and families on therapeutic exercises, precautions and chronic condition management.
- Collaborate with other therapists, nurses, and social workers to optimize care plans and outcomes.

Requirements
- Active Physical Therapist license in California or eligibility
- Experience planning and conducting therapy segments and working within an interdisciplinary team
- Ability to work in a home healthcare - hospice setting
- Strong communication and collaboration skills, with clear documentation
- Availability for a 13-week travel assignment beginning 04/27/2026
